PLEASE PROVIDE THE DETAILS OF ALL OPTOMETRISTS YOU WISH TO BE GIVEN ACCESS TO THE OPTICAL EXPRESS PARTNER SYSTEM.

We will register your practice and each accredited optometrist will be given their own personal access to our secure electronic portal to complete and submit a co-management exam report for Optical Express post surgery patients. Once your practice has been set up our Community Relationship Manager for your area will contact you to arrange a convenient time to visit to go through the system and invoicing procedure. When your referred patients have had surgery we will send an outcome letter to your practice and to the patient, advising them to have a post surgery check. Only patients you have referred will be directed to your practice. The post operative check should take place after 4 weeks no sooner. Once submitted our clinical team will then make a decision on any further needs of the patient or they will be discharged.

2. Patient attends a Pre-Assessment at an Optical Express hospital or clinic.

3. Patient is booked for surgery at an Optical Express treatment hospital or clinic.

We use the ODS Code for practices, provided by the NHS Organisation Data Service. You can check your ODS code by using the NHS ODS search tool.

For Practices In Scotland

We use the Public Health Scotland‘s Practice List Number for practices.
You can check your Practice List Number by using obtaining the latest “optprac” list and finding your practice.
