Our Co-Management Portal uses two sources of unique identification codes for practices –
We use the ODS Code for practices, provided by the NHS Organisation Data Service. You can check your ODS code by using the NHS ODS search tool.
We use Public Health Scotland‘s Practice List Number for practice identification.
You can check your Practice List Number by downloading the latest “optprac” list. After downloading the csv file using the button below, open this file in your spreadsheet software (e.g. MS Excel). Your Practice List Number is in the 2nd column of the row that shows your practice.
